Implementation evaluation of nurse-initiated care in 29 Australian emergency departments: a RE-AIM multi-method evaluation.

Abstract
Remote patient monitoring plays a role in managing chronic medical conditions, including chronic kidney disease (CKD) and home-based dialysis therapies such as peritoneal dialysis (PD) [–]. The importance of remote health monitoring has increased substantially since the COVID-19 pandemic. Numerous studies have highlighted its benefits for patients with CKD, including those on dialysis, through monitoring and feedback [–]. These advantages include better blood pressure control, reduced reliance on antihypertensive medications, fewer emergency clinic or hospital visits, and improved patient quality of life [, , ]. A previous review identified approximately 13 mobile applications designed to aid CKD and kidney failure patients in tracking medication compliance and nutrition []. However, these apps lacked comprehensive features addressing health-related parameters or dialysis-specific information. Another review of apps for PD patients found that only a few out of 14 apps effectively catered to the unique needs of this population []. Most research has focused on PD patients, particularly those using automated peritoneal dialysis (APD) [, ]. Currently, no app provides comprehensive support for both CKD and PD patients, integrating features such as vital sign recording, medication tracking with reminders, nutritional logs, dialysis-specific parameters, and alerts for inactive usage. This highlights the need for a well-designed app that serves as a holistic tool for patient management. Thus, the main outcome of the study was to assess the acceptance of the Viva Konnect patient app [].
