Abstract

A short Behavior Change Training module can be utilized to enhance confidence, knowledge, and skills of allied healthcare professionals, enabling them to effectively integrate and deliver behavior change advice within transition care services. Policymakers can embed behavior change training into standard professional development programs across transition care sites. This approach has been successfully implemented with other programs in Australia and internationally (such as the Making Every Contact Count initiative in the UK). Future research should evaluate the long-term impact of behavior change training on staff behavior and client outcomes, including hospital readmission rates and functional independence, using implementation science frameworks to guide scale-up and sustainability.
