Abstract

California exhibits a favorable health profile compared to the national average. In 2021, California recorded the 45th-highest age-adjusted death rate, indicating a lower overall mortality burden than most other states [, ]. This improved mortality picture is further supported by the state's heart disease death rate, which was 147.8 per 100,000 residents in 2021, nearly 15% lower than the national average of 173.8 [, ]. Additionally, California ranks as the 4th-highest state for life expectancy at birth in the U.S., with newborns expected to live 79.0 years, a full 2.0 years longer than the national average [, ]. These statistics establish California as a state with generally robust health indicators. However, certain regions in California, such as Fresno County and the overarching Central Valley, have demonstrated alarming disparities compared to California's overall health profile. This situation is not merely an isolated issue of poor health outcomes in one county; rather, it indicates that Fresno County's health statistics actively diminish the regional and statewide averages. This phenomenon highlights a significant intra-state equity challenge. California’s overall health success is not equitably shared across its diverse communities.
