Abstract

Recently, [F]­MC225 () has been approved
in two different clinical
trials as a PET 18-F radiotracer, in human, Phase II, because all
ADME described properties in in vivo studies are largely satisfied.
As reported in European Portal Ctis, Luurtsema et al. are investigating the involvement of -glycoprotein
at the BBB level in Alzheimer and Parkinson patients. The expected
results should demonstrate that in Alzheimer patients the uptake of
the PET radiotracer [F]­MC225 is more conspicuous than
in the control group because in these patients there is a loss of -gp activity and expression at the BBB level. -glycoprotein is involved in efflux of many drugs and of [F]­MC225, which is a weak -gp substrate. In the recently approved clinical trial study, it was demonstrated that in patients being treated with
antidepressant drugs the brain uptake of PET radiotracer should be
more abundant that in patients that are not responsive to antidepressant
drugs because the hypothesis is that there is an induced overexpression
of -gp in the nonresponsive patients. In these studies, the control group is the first whereas the second
group of patients should demonstrate a reduced brain uptake of radiotracer
in PET because the nonresponsive state seems to be due, as reported
in several papers, to overexpression and/or over-activity of -glycoprotein at the BBB level. Summarizing this finding, [F]­MC225
is the first radioligand
PET approved in two different clinical trials that after the studies
of Phase I, in humans, carried out by Toyohara et al., is useful to in vivo targeting -glycoprotein at the BBB level. These studies should lead to employing
the promising radiotracer [F]­MC225, to give early diagnosis
in CNS pathologies such as AD, PD, and MCI. Moreover, we defined if -gp is involved in drug-resistant treatment in a specific
class of antidepressive drugs.
